Annual Child Packet Update Forms *This is the packet you need to fill out completely and return to the GLC administrative office to update your child's GLC annual enrollment information. This should be completed & submitted between August 10th-20th each year. If you haven't completed one for your child yet, please do so as soon as possible!
Our required ODJFS Child Enrollment and Health Information Form. This form is used for all new or annual enrollments. THIS form may be filled in online but must be printed because it cannot be saved or submitted online.
